Rehabilitation Case Manager

We are currently looking to recruit 2 full-time Rehabilitation Complex Case Managers with a recognised clinical qualification in Physiotherapy, Occupational Therapy or Nursing. The role is to cover the north of the UK with a mixture of home visits and remote visits using skype and telephone calls.

 

We seek a Rehabilitation Case Manager who has a belief and understanding of the value of integrating medical services and return to work activities within a client’s rehabilitation program. The role will involve:

 

  • Conducting Immediate Needs Assessments with clients face-to-face in their homes
  • Assessment of a client’s physical and psychological capacity; identification of barriers to recovery; and development of effective rehabilitation goals and plans
  • Communication amongst clients, medical and health practitioners, and other stakeholders
  • Vocational rehabilitation; returning clients to meaningful work with their same or new employer
  • Implementation and management of a rehabilitation programme including cost control
  • Completion of rehabilitation update reports
  • Adherence to strict SLA’s and Key Performance Indicators

 

The salary for the position is negotiable depending on experience.

Occupational Health Physician - Nottingham

Based in Nottingham duties will include but not be limited to:

  • Effective provision of medical services to client companies by the Clinical Team.
  • Acting as CMO for clients advising them on policy, general health advice and HR/absence management strategies.

The salary for the role is from £75,000 - £120,000 depending on experience (pro rata for part-time).
